ABSTRACT

ABSTRACT Objective: To investigate exclusive breastfeeding (EBF) intention and associated variables among women in the third trimester of pregnancy. Methods: The data were collected with a questionnaire for the pregnant women (n=653), from December/2018 to November/2019. They answered the Infant Feeding Intentions (IFI) scale, translated and adapted to Brazilian Portuguese, and a questionnaire on sociodemographic, biological, family, pregnancy, breastfeeding, health care, and habits variables. Descriptive statistics and multiple logistic regression analyses were performed with a 5% significance level, following a multilevel hierarchical model that estimated the association between the dependent and independent variables. The outcome EBF intention measured by the IFI score was dichotomized by the median (<16 or =16). Results: Mean±standard deviation score for the IFI scale was 14.4±2.6 (score 0: very strong intention to not breastfeed at all; score 16: very strong EBF intention up to six months). The results from the regression analysis showed that pregnant women who had no intention to bottle feed (OR=4.33; 95%CI 2.79-6.72) or did not know (OR=1.85; 95%CI 1.21-2.82), those who planned the pregnancy (OR=1.52; 95%CI 1.09-2.12), those who believed they would have help to care for the baby (OR=3.60; 95%CI 1.51-8.56) or did not know (OR=3.97; 95%CI 1.26-12.51), and those who reported knowing the World Health Organization recommendations on breastfeeding (OR=1.73; 95%CI 1.13-2.64) were more likely to show a very strong EBF intention. Conclusions: Pregnant women in the third trimester of pregnancy presented a strong EBF intention. The higher EBF intention score was significantly associated with the structural, setting, and individual determinants.


RESUMO Objetivo: Investigar a intenção materna de amamentar exclusivamente (IMA) e variáveis associadas entre as mulheres no terceiro trimestre de gravidez. Métodos: A coleta de dados foi realizada por meio de entrevista e questionário semiestruturado e autoadministrado, entre dezembro/2018 e novembro/2019. As gestantes (n=653) responderam à escala Infant Feeding Intentions (IFI) testada, traduzida e adaptada para o português do Brasil e ao questionário contendo perguntas sobre variáveis sociodemográficas, biológicas e familiares, bem como relacionadas à gestação, ao aleitamento materno, à assistência à saúde e a hábitos. Foram realizadas análises estatísticas descritivas e de regressão logística múltipla hierarquizada, com nível de significância de 5%, para estimar a associação entre as variáveis independentes e o desfecho. A IMA, medida pela escala IFI, foi dicotomizada pela mediana (<16 ou =16). Resultados: A pontuação média±desvio padrão para a escala IFI foi de 14,4±2,6. Os resultados da análise de regressão mostraram que as gestantes que não tinham intenção de oferecer mamadeira (OR=4,33; IC95% 2,79-6,72) ou não sabiam (OR=1,85; IC95% 1,21-2,82), que planejaram a gestação (OR=1,52; IC95% 1,09-2,12), aquelas que acreditavam que teriam ajuda nos cuidados com o bebê (OR=3,60; IC95% 1,51-8,56) ou que não sabiam (OR=3,97; IC95% 1,26-12,51), bem como aquelas que relataram conhecer as recomendações da Organização Mundial de Saúde sobre amamentação (OR=1,73; IC95% 1.13-2.64) tinham mais chances de mostrar uma IMA muito forte. Conclusões: As gestantes no terceiro trimestre de gestação apresentaram forte IMA. A maior IMA esteve significativamente associada aos determinantes estruturais, contextuais e individuais.

ABSTRACT BACKGROUND: Exclusive breastfeeding is recommended for the first six months, and mother's age impact early weaning. Educational support and relevant information can increase breastfeeding rates. OBJECTIVE: To determine whether antenatal education enhances the maintenance, intention, and confidence in breastfeeding among adolescents. DESIGN AND SETTING: A prospective cohort study involving primiparous adolescents who gave birth at the Woman's Hospital (CAISM), Universidade Estadual de Campinas, Brazil. METHODS: Adolescent mothers were categorized into two groups based on the location of prenatal care: those at the Woman's Hospital (WH) who received antenatal education, and at the Primary Care (PC) who did not receive antenatal education. All adolescents received breastfeeding orientation during their postpartum hospital stay. The groups were compared using the Student's t-test, Mann-Whitney U test, and chi-squared test. Log-binomial models were used to compare the groups at different time intervals. RESULTS: The study included 132 adolescents: 59 in the WH group and 73 in the PC group. Six months postpartum, adolescents in the WH group demonstrated higher engagement in breastfeeding (P < 0.005) and exclusive breastfeeding (P = 0.04) than PC group. PC group showed greater lack of confidence in breastfeeding (P = 0.02) and felt less prepared (P = 0.01). Notably, all WH adolescents reported a stronger desire to breastfeed after antenatal education. CONCLUSION: Antenatal education significantly improves the maintenance, intention, and confidence of breastfeeding among adolescents. This education approach can be implemented across all healthcare levels and should be made accessible to all women throughout the pregnancy and postpartum period.

ABSTRACT

RESUMO Objetivo Comparar a introdução das consistências no período da alimentação complementar de recém-nascidos prematuros e recém-nascidos a termo até os 12 meses de vida, bem como avaliar a presença de disfunção motora oral e a sua relação com dificuldade na introdução das consistências alimentares nestes grupos. Método Trata-se de um estudo do tipo observacional, analítico, coorte, com coleta de dados ambispectiva, realizado na Secretaria Municipal de Saúde de Mafra - SC. A amostra do estudo foi constituída de 87 recém-nascidos, sendo 41 a termo e 46 prematuros. Na coleta de dados, foram realizadas entrevistas com as mães/responsáveis. A avaliação antropométrica foi realizada por nutricionista por meio de mensuração do peso corporal, comprimento e perímetro cefálico, seguida de avaliação das habilidades motoras orais e funcionais por meio do PAD-PED adaptado, avaliação da mamada, avaliação do desenvolvimento neuropsicomotor, avaliação da presença de depressão materna e risco psíquico da criança, até os 12 meses de vida de idade corrigida. Resultados A disfunção motora oral foi observada em 15 recém-nascidos, em ambos os grupos, na consistência líquida na primeira avaliação, persistiu em dois casos nos recém-nascidos a termo e em três casos nos recém-nascidos pré-termo, na última avaliação para a consistência sólida. Conclusão Não foi observada diferença na introdução das consistências alimentares entre os grupos. O aleitamento materno foi mais frequente nos a termo na primeira avaliação e semelhante nas demais avaliações. Com relação aos preditivos para disfunção motora oral, a mamadeira aumentou a chance em cerca de 7 vezes e procedimentos orais invasivos cerca de 6 vezes.


ABSTRACT Purpose To compare the introduction of consistencies during the period of complementary feeding of preterm and full-term newborns up to 12 months of life, as well as to evaluate the presence of oral motor dysfunction and its relation to difficulty in introducing food consistencies in these groups. Methods This is an observational, analytical, cohort study, with ambispective data collection, carried out at the Municipal Department of Health of Mafra, state of Santa Catarina, Brazil. The study sample consisted of 87 newborns, 41 full-term and 46 preterm. While data was collected, interviews were held with the mothers/guardians. The anthropometric assessment was carried out by a nutritionist by measuring body weight, length, and head circumference, followed by assessment of oral and functional motor skills by the adapted Clinical Evaluation Protocol of Pediatric Dysphagia (PAD-PED), assessment of breastfeeding and neuropsychomotor development, and assessment of the presence of maternal depression and psychological risk of children with up to 12 months of corrected age. Results We verified oral motor dysfunction in 15 newborns, in both groups, in the liquid consistency in the first assessment, persisting in two cases in the full-term newborns and in three cases in the preterm infants, in the last assessment for the solid consistency. Conclusion We observed no difference in the introduction of food consistencies between groups. Breastfeeding was more frequent in newborns in the first assessment and similar in other assessments. Regarding the predictors for oral motor dysfunction, bottle feeding increased the odds by about seven times and invasive oral procedures by about six times.

ABSTRACT

Objetivo: revisar e validar os elementos do Diagnóstico de Enfermagem "Amamentação ineficaz" (00104) a partir da literatura e consenso de especialistas e construir definições operacionais para suas características definidoras. Método: trata-se de uma pesquisa metodológica desenvolvida em duas fases : revisão de escopo, baseada no proposto pelo Joanna Briggs Institute, e validação de conteúdo. Foram considerados validados os itens com Índice de Validade de Conteúdo ≥ 0,80 quanto a relevância, clareza e precisão. Resultados: elementos do Diagnósticos de Enfermagem foram mantidos como estão na atual edição da Classificação de Diagnósticos de Enfermagem da NANDA International (NANDA-I), enquanto outros sofreram modificações. Além disso, foi proposta a inclusão de fatores relacionados, populações de risco e condições associadas. Conclusão: este estudo possibilitou revisão e validação de conteúdo do Diagnóstico de Enfermagem "Amamentação ineficaz", presente na Classificação dos Diagnósticos de Enfermagem da NANDA-I


Objective: to review and validate the elements of the Nursing Diagnosis "Ineffective Breastfeeding" (00104) based on the literature and expert consensus, and to construct operational definitions for its defining characteristics. Method: this is a methodological study carried out in two phases: a scoping review, based on that proposed by the Joanna Briggs Institute, and content validation. Items with a Content Validity Index ≥ 0.80 in terms of relevance, clarity and precision were considered validated. Results: elements of the Nursing Diagnoses were kept as they are in the current edition of the NANDA International Classification of Nursing Diagnoses (NANDA-I), while others were modified. In addition, the inclusion of related factors, risk populations and associated conditions was proposed. Conclusion: this study enabled a review and validation of the content of the Nursing Diagnosis "Ineffective breastfeeding", present in the NANDA-I Classification of Nursing Diagnoses

Abstract Objectives To evaluate outcomes of oral food challenge (OFC) test to assess tolerance in infants with non-IgE-mediated cow's milk allergy (CMA) with gastrointestinal manifestations and explore clinical data predictive of these outcomes. Methods Single-center retrospective study including infants (age < 12 months) who were referred for CMA between 2000 and 2018 and underwent OFC on follow-up. A univariate logistic regression test was performed to evaluate variables associated with the outcomes of the follow-up OFC test. Results Eighty-two patients were included, 50% were male. Eighteen patients had a positive OFC test (22%). Most patients had presented with hematochezia (77%). The median age of symptom onset was 30 days. Two-thirds of the patients were on appropriate infant formula (extensively hydrolyzed or amino acid-based formula), exclusively or in association with breastfeeding. The median time on an elimination diet before the OFC test was 8 months (Q1 6 - Q3 11 months). All cases with positive follow-up OFC tests (n= 18) had been exposed to cow's milk-based formula before the first clinical manifestation of CMA. Five out of eight cases with Food Protein-Induced Enterocolitis Syndrome (FPIES) had positive OFC tests. Exposure to cow's milk-based formula before diagnosis, a history of other food allergies, hematochezia and diarrhea were predictors of a positive OFC test. Conclusions In infants with non-IgE-mediated CMPA with gastrointestinal manifestations, the use of cow's milk-based formula, a history of other food allergies, and hematochezia and diarrhea upon initial presentation were associated factors for the later achievement of tolerance.

SUMMARY OBJECTIVE: The aim of this study was to evaluate the postpartum hemorrhage, perineal integrity, and breastfeeding results of mothers who underwent oxytocin induction in the first stage of labor in the early postpartum period. METHODS: This single-center observational case-control study was conducted in the obstetric unit of a public hospital in Istanbul. The study sampling included 44 pregnant women who received oxytocin induction (case group) and 44 pregnant women who did not receive oxytocin (control group). The Personal Information Form, LATCH Breastfeeding Assessment Tool, Breastfeeding Self-Efficacy Scale, Redness, Edema, Ecchymosis, Discharge, and Approximation Scale, and Postpartum Hemorrhage Collection Bag were used in data collection, and pad follow-up was carried out. RESULTS: The amount of hemorrhage in the first 24 h of the postpartum period and the mean Redness, Edema, Ecchymosis, Discharge, and Approximation Scale score were significantly higher in the case group. While 47.7% of the oxytocin-induced women had 1st or 2nd, and 11.4% had 3rd or 4th degrees of lacerations, 20.5% of the control group had 1st or 2nd, and 2.3% had 3rd or 4th degrees of lacerations. There was no significant difference between the mean scores of the Breastfeeding Self-Efficacy Scale and LATCH Breastfeeding Assessment Tool in both groups. CONCLUSION: According to the study findings, it was determined that oxytocin induction administered in the first stage of labor increased hemorrhage and perineal trauma in the early postpartum period but did not affect the results of breastfeeding. ABSTRACT Objectives To identify the prevalence and determinants of continued breastfeeding in Haitian children aged 12-23 months. Methods Three cross-sectional surveys were conducted yearly during the summers of 2017 to 2019 as part of a 4-year (2016-2020) multisectoral maternal and infant health initiative in the regions of Les Cayes, Jérémie, and Anse d'Hainault in Haiti. A total of 455 children 12-23 months of age and their mothers participated in the study. A child was considered to be continuing breastfeeding if the mother reported giving breast milk in the 24-hour dietary recall. Unadjusted and adjusted prevalence ratios were estimated, and associations were assessed between continued breastfeeding and explanatory factors related to sociodemographic characteristics, household food security, maternal nutrition, and breastfeeding knowledge and practices. Results The prevalence of continued breastfeeding was 45.8%. Continued breastfeeding was significantly more prevalent among younger children, children who did not have a younger sibling, children whose mother was not pregnant, those living in the Jérémie region, children who had been exclusively breastfed for less than 1 month, and children whose mother knew the World Health Organization's recommendation for continued breastfeeding up to 2 years or beyond. Conclusions The study results highlight the need for geographically equitable access to tailored and adequate health services and education that support breastfeeding in a way that is compatible with the local context.


RESUMEN Objetivos. Determinar la prevalencia y los determinantes asociados a la continuación de la lactancia materna en la población infantil haitiana de entre 12 y 23 meses. Métodos. Durante los veranos del 2017 al 2019 se llevaron a cabo tres encuestas transversales anuales como parte de una iniciativa multisectorial de salud materna e infantil de 4 años (2016 a 2020) en las regiones de Les Cayes, Jérémie y Anse d'Hainault de Haití. En el estudio participaron 455 menores de edades comprendidas entre 12 y 23 meses y sus madres. Se consideró que un menor continuaba con la lactancia materna si, en la evaluación de la alimentación basada en el recuerdo de 24 horas, la madre declaraba la toma de leche materna. Se calcularon los cocientes de prevalencia ajustados y sin ajustar y se evaluó la asociación entre la continuación de la lactancia materna y posibles factores explicativos relacionados con las características sociodemográficas, la seguridad alimentaria de la familia, la nutrición materna y los conocimientos y prácticas en materia lactancia materna. Resultados. La prevalencia de la continuación de la lactancia materna fue del 45,8%. Esta prevalencia fue significativamente mayor cuando se trataba de lactantes de menor edad, cuando no había hermanos menores, cuando la madre no estaba embarazada, en los residentes en la región de Jérémie, cuando había habido una alimentación exclusiva con leche materna durante menos de un mes y cuando la madre conocía la recomendación de la Organización Mundial de la Salud de continuar con la lactancia materna hasta los dos años o más. Conclusiones. Los resultados del estudio ponen de relieve la necesidad de disponer de un acceso geográfico equitativo a unos servicios de salud y una educación que respalden la lactancia materna de una manera compatible con el contexto local.


RESUMO Objetivos Identificar a prevalência e os determinantes do aleitamento materno continuado em crianças haitianas de 12 a 23 meses de idade. Métodos Três pesquisas transversais foram realizadas anualmente nos verões de 2017 a 2019 como parte de uma iniciativa multissetorial de saúde materno-infantil de quatro anos (2016-2020) nas regiões de Les Cayes, Jérémie e Anse d'Hainault no Haiti. Um total de 455 crianças de 12 a 23 meses de idade e suas mães participaram do estudo. Considerou-se que a criança continuava sendo amamentada se a mãe relatasse ter dado leite materno no recordatório alimentar de 24 horas. Foram estimadas as taxas de prevalência não ajustadas e ajustadas e foram avaliadas as associações entre o aleitamento materno continuado e os fatores explicativos relacionados às características sociodemográficas, à segurança alimentar da família, à nutrição materna e aos conhecimentos e práticas de aleitamento materno. Resultados A prevalência de aleitamento materno continuado foi de 45,8%. O aleitamento materno continuado foi significativamente mais prevalente entre crianças mais novas, crianças que não tinham irmão mais novo, crianças cuja mãe não estava grávida, residentes da região de Jérémie, crianças que haviam sido exclusivamente amamentadas por menos de um mês e crianças cuja mãe conhecia a recomendação da Organização Mundial da Saúde de manter o aleitamento materno por 2 anos ou mais. Conclusões Os resultados do estudo destacam a necessidade de acesso geograficamente equitativo a educação e serviços de saúde adaptados e adequados para apoiar o aleitamento materno de forma compatível com o contexto local.

Objetivo: Conhecer as tecnologias educacionais voltadas à prática do aleitamento materno desenvolvidas por enfermeiros. Método: Revisão Integrativa realizada nas bases de dados da Coordenação de Aperfeiçoamento de Pessoal de Nível Superior e Biblioteca Digital Brasileira de Teses e Dissertações, em dezembro de 2021. Inicialmente foram identificados 313 trabalhos, após aplicados critérios de inclusão e exclusão foram selecionados 13 estudos para a amostra. Resultados: Verificou-se as seguintes tecnologias: álbum seriado, iconografia 3D, rádio, telefone, literatura de cordel, websites e simuladores realísticos de baixa fidelidade. Os estudos visaram desenvolver, avaliar ou validar tecnologias educacionais voltadas para o aleitamento materno no que diz respeito aos benefícios quando se trata do manejo correto. Conclusão: As tecnologias educacionais têm sido alvo no desenvolvimento de pesquisas em Enfermagem, contribuindo para a promoção do aleitamento materno, beneficiando mães e bebês e dinamizando o processo de cuidar.


Objective: To learn about educational technologies related to breastfeeding practice. Methods: Integrative Review conducted in the Coordination for the Improvement of Higher Education Personnel (CAPES) and Brazilian Digital Library of Theses and Dissertations (BDTD) databases, from December 2019 to December 2021. Initially, 313 studies were identified, and after applying the inclusion and exclusion criteria, 13 studies were selected for the sample. Results: The following technologies were verified: serialized album, 3D iconography, radio, telephone, string literature, websites, and low fidelity realistic simulators. The studies aimed to develop, evaluate, or validate educational technologies focused on breastfeeding with regard to the benefits when it comes to the correct management. Conclusion: Educational technologies have been a target in the development of nursing research, contributing to the promotion of breastfeeding, benefiting mothers and babies and streamlining the care process.

Métodos: revisión integradora realizada en las bases de datos Coordinación para el Perfeccionamiento del Personal de Educación Superior (CAPES) y la Biblioteca Digital Brasileña de Tesis y Disertaciones (BDTD), desde diciembre de 2019 hasta diciembre de 2021. Inicialmente se identificaron 313 estudios, luego de aplicar los criterios de inclusión y exclusión, se seleccionaron 13 estudios para la muestra. Resultados: se verificaron las siguientes tecnologías: álbum en serie, iconografía 3D, radio, teléfono, literatura de cuerdas, sitios web y simuladores realistas de baja fidelidad. Los estudios tuvieron como objetivo desarrollar, evaluar o validar tecnologías educativas orientadas a la lactancia materna en cuanto a beneficios en cuanto a un correcto manejo se refiere. Conclusión: Las tecnologías educativas han sido un objetivo en el desarrollo de la investigación en enfermería, contribuyendo a la promoción de la lactancia materna, beneficiando a madres y bebés y agilizando el proceso de atención.

ABSTRACT

La glándula mamaria tiene su origen embriológico, posteriormente se diferencia histológica y anatómicamente; a lo largo del tiempo se han ido descubriendo nuevos hallazgos, además de nueva terminología. El objetivo de este trabajo consistió en describir la actualización en los aspectos morfológicos de la glándula mamaria. Se realizó una búsqueda de artículos en diferentes fuentes primarias y secundarias de la literatura científica como: Pubmed, Embase, WOS, Scopus, Scielo obteniendo 623 publicaciones, seleccionado 53 para su revisión y 17 incluidos. La mama se ha ido actualizando en los aspectos morfológicos; Los conductos se ubican cercanos a la areola y al pezón donde desembocan directamente, las ramificaciones de los conductos se ubican cercanos a la areola; los pezones tienen entre 15 y 20 lóbulos, de los cuales sólo 5-9 son verdaderos orificios del conducto mamario, no existen senos lactíferos por lo que la leche desemboca directamente en el pezón, en cuanto a la inervación de la mama son inervados por las ramas cutáneas anterior y lateral de los nervios intercostales. La evidencia existente demuestra la necesidad de realizar estudios permanentes en el ámbito de la morfología, como fue descrito en este artículo de revisión sobre la mama. En diferentes aspectos, tales como términos, funciones y estructuras. Está información es importante para, la formación de pregrado, post grado, y su utilización en la clínica, identificación de patologías, clínicas de lactancia, entre otros.


SUMMARY: The mammary gland has its embryological origin, later it differentiates histologically and anatomically; over time new findings have been discovered, as well as new terminology. The aim of this work was to describe the update on the morphological aspects of the mammary gland. A search for articles was carried out in different primary and secondary sources of scientific literature such as: Pubmed, Embase, WOS, Scopus, Scielo, obtaining 623 publications, 53 selected for review and 17 included. The breast has been updated in morphological aspects; the ducts are located close to the areola and the nipple where they open directly, the branches of the ducts are located close to the areola; the nipples have between 15 and 20 lobes, of which only 5- 9 are true mammary duct orifices, there are no lactiferous sinuses, so the milk flows directly into the nipple, in terms of innervation of the breast they are innervated by the anterior and lateral cutaneous branches of the intercostal nerves. The existing evidence demonstrates the need for permanent studies in the field of morphology, as described in this review article on the breast. In different aspects, such as terms, functions and structures. This information is important for undergraduate and postgraduate training, and its use in the clinic, identification of pathologies, lactation clinics, among others.

ABSTRACT

Objetivos: Comprender las experiencias sobre la lactancia materna desde la perspectiva de las mujeres, en un grupo de madres lactantes primerizas. Materiales y métodos: Estudio cualitativo, bajo la perspectiva de etnografía, enfocado en un grupo de mujeres de la región oriente del departamento de Antioquia (Colombia). Por muestreo intencional y hasta la saturación de los datos se seleccionaron 16 mujeres lactantes primerizas, de 18 años o más, con hijos menores de 24 meses y que se encontraran amantando o hubiesen amantado de forma exclusiva al menos durante tres meses. Se realizaron entrevistas semiestructuradas y observación participante; el análisis de los datos se desarrolló por triangulación y codificación abierta y axial. Los resultados se presentan a través de la descripción de categorías. Resultados: Se identificaron cuatro categorías frente a la experiencia del amamantamiento: el agotamiento y el dolor que se disipan con amor, la lactancia y el nuevo orden de las cosas, las emociones de una mujer en el cuerpo de una madre y la lactancia: un acto social. Conclusiones: Proteger la lactancia materna requiere poner especial cuidado a la mujer como sujeto, más allá del amamantamiento, dando prioridad a su salud física y emocional como elementos esenciales que deben ser considerados por los profesionales en salud. Se requieren estudios que evalúen los efectos de la creación de grupos de apoyo que propicien el intercambio de saberes para acompañar a la mujer lactante


Objectives: To understand the breastfeeding experience from the perspective of a group of women who breastfeed for the first time. Material and methods: Qualitative study from an ethnographic perspective, focusing on a group of women from the eastern region of the Department of Antioquia (Colombia). Intentional sampling until data saturation was used. The group included 16 first-time breastfeeding mothers aged 18 or older, with children under 24 months of age and who were breastfeeding or had breastfed exclusively for at least three months. Semi-structured interviews and participant observations were carried out. Data were analyzed by triangulation and open and axial coding. The results are presented in the form of category descriptions. Results: Four categories were identified in relation to the breastfeeding experience: exhaustion and pain dispelled by love, breastfeeding and the new order of things, emotions of a woman embodied in a mother, and breastfeeding as a social act. Conclusions: Protection of breastfeeding requires special attention to the woman as a subject beyond the act of breastfeeding, giving priority to her physical and emotional health as essential elements that must be kept in mind by healthcare professionals. Studies to assess the effects of creating support groups that foster the exchange of insights to help the breastfeeding woman are required.

ABSTRACT

La lactancia materna es fundamental para la salud del infante y se ve influida por diversos factores, entre ellos la salud mental materna. En particular, las madres que tienen síntomas depresivos tienen mayor riesgo de presentar dificultades de lactancia y de interrumpir tempranamente la lactancia exclusiva y la lactancia en general. Por otra parte, la lactancia materna actúa como un factor protector de la salud mental materna en algunas circunstancias, en tanto las dificultades de lactancia tienen un impacto negativo en la salud mental de la mujer. La presente revisión describe algunos de los mecanismos fisiológicos que subyacen al establecimiento y la mantención de la lactancia, asociados a la prolactina, la oxitocina, la dopamina y la serotonina, así como a la experiencia de la lactancia y la presencia de dificultades en esta área, y como estas interactúan con las dificultades emocionales de la madre. Se ofrece un modelo integrativo que considera aspectos hormonales y fisiológicos para comprender la asociación compleja y bidireccional entre el establecimiento de una lactancia exitosa y la salud mental materna.


Breastfeeding is essential for infant health and development. It is influenced by multiple factors, including maternal mental health. In particular, mothers who present depressive symptoms are at greater risk of presenting breastfeeding difficulties and presenting shorter exclusive breastfeeding and breastfeeding in general. On the other hand, breastfeeding acts as a protective factor for maternal mental health in some circumstances. Also, breastfeeding difficulties have a negative impact on womens mental health. This review describes some of the physiological mechanisms underlying the establishment and maintenance of lactation, associated with prolactin, oxytocin, dopamine, and serotonin. As well as how the lactation experience and the presence of difficulties in this area interact with the mothers emotional functioning. An integrative model is proposed, which considers hormonal and physiological aspects involved in the complex and bidirectional association between breastfeeding successful establishment and maternal mental health.

ABSTRACT

Se considera fundamental el acompañamiento oportuno y actualizado del equipo de salud en el inicio y mantenimiento de la lactancia materna. Se ha documentado que algunos profesionales carecen de habilidades para ofrecer orientación adecuada a las personas lactantes, pero no existen pesquisas locales con instrumentos estandarizados acerca de sus conocimientos. Adaptar un cuestionario validado permitiría indagar y comparar estos datos con otros países, y justificaría la implementación de las estrategias educativas necesarias. El objetivo fue obtener una versión argentina de la Encuesta de Conocimientos en Lactancia (ECoLa) mediante un proceso de adaptación transcultural mediado por la participación de lingüistas, expertos en lactancia, y profesionales de medicina y enfermería como destinatarios del instrumento. La adecuación, equivalencia y comprensión fueron comprobadas en los discursos de expertos y autores, y en las respuestas de las entrevistas cognitivas. Se consiguió una versión de ECoLa equivalente a la versión original; semántica y culturalmente adaptada a profesionales argentinos.


A timely and updated support from the health care team in the initiation and maintenance of breastfeeding is essential. It has been documented that some health care providers lack the skills to offer adequate guidance to nursing mothers, but there are no local surveys on their knowledge based on standardized instruments. Adapting a validated questionnaire would allow to investigate and compare these data with those of other countries, and support the implementation of necessary educational strategies. Our objective was to obtain an Argentine version of the Survey on Breastfeeding Knowledge (Encuesta de Conocimientos en Lactancia, ECoLa) through a cross-cultural adaptation process with linguists, breastfeeding experts, physicians, and nurses. Adequacy, equivalence, and comprehension were verified in the discourses of experts and authors and cognitive interviews. A version of the ECoLa equivalent to the original version was obtained, which was semantically and culturally adapted to Argentine health care providers.

ABSTRACT

Objetivos: Describir la prevalencia del riesgo de deficiencia o exceso en la ingesta de nutrientes y el patrón de consumo de alimentos de acuerdo con las recomendaciones establecidas por el Ministerio de Salud y Protección Social y el Instituto Colombiano de Bienestar Familiar. Materiales y métodos: Se realizó un estudio transversal descriptivo en mujeres sanas que fueron atendidas en dos hospitales del oriente antioqueño, con seguridad alimentaria y nutricional (SAN) según la Escala Latinoamericana y Caribeña de Seguridad Alimentaria (ELCSA), en el primer trimestre de lactancia, evaluadas entre los años 2021 y 2022. La fuente de información fue la historia clínica. Se midieron las características sociodemográficas, clínicas, antropométricas y la ingesta dietética y de micronutrientes. Se empleó estadística descriptiva. Se presenta la prevalencia de déficit o exceso de ingesta calórica y el índice de relación de consumo (IRC). Se utilizaron los software EVINDI v5, PC-SIDE v1.0, Stata 16.1 y Jasp 0.16.4. Resultados: Se incluyeron 30 mujeres lactantes. La prevalencia de riesgo de deficiencia en la ingesta usual de energía fue 43 % y exceso 16 %; el riesgo de deficiencia proteica fue 98 %. El consumo superior al valor de referencia para grasa saturada fue 86 % y carbohidratos simples 72 %. El patrón de consumo se caracterizó por superar las recomendaciones para azúcares (IRC = 1,29), leche y derivados (IRC = 1,09), cereales, raíces, plátanos y tubérculos (IRC = 1,04). No cumplieron las recomendaciones en la ingesta de grasas (IRC = 0,70), carnes, huevos, leguminosas, frutos secos y semillas (IRC = 0,49), frutas y verduras (IRC = 0,41). Conclusiones: El patrón alimentario identificado dista de las guías nacionales, lo que limita el consumo de macro y micronutrientes, y contribuye al círculo intergeneracional de la malnutrición. Es fundamental realizar nuevas investigaciones en el país para identificar otros patrones de consumo e impulsar acciones de política al respecto.


Objectives: To describe the prevalence of the risk of under or overnutrition and the food intake pattern vis á vis the recommendations of the Ministry of Health and Social Protection and the Colombian Family Welfare Institute. Material and methods: Descriptive cross-sectional study carried out in healthy women receiving care in two hospitals of eastern Antioquia, with food and nutrition security in accordance with the Latin American and Caribbean Food Security Scale (ELCSA), in the first trimester of breastfeeding, assessed between 2021 and 2022. The clinical record was used as the source of information. The sociodemographic, clinical, anthropometric characteristics, as well as dietary and micronutrient intake, were measured. Descriptive statistics were used. The prevalence of deficient or excessive energy intake and the food consumption score (FCS) are presented. The EVINDI v5, PC-SIDE v1.0, Stata 16.1 and Jasp 0.16.4 software packages were used. Results: Overall, 30 breastfeeding women were included. The prevalence of the risk of deficient energy intake was 43 %, while the risk of excessive intake was 16 % and the risk of protein deficiency was 98 %. Intake exceeding the reference value for saturated fats was 86 %, and 72 % for simple carbohydrates. The consumption pattern was characterized by exceeding the recommendations for sugars (FCS = 1.29), milk and dairy products (FCS = 1.09), grains, roots, plantains and tubers (FCS = 1.04). Recommendations for the intake of fats (FCS = 0.70), meats, eggs, legumes, nuts, seeds (FCS = 0.49), fruits and vegetables (FCS = 0.41) were not met. Conclusions: The food intake pattern identified is far from meeting the national guidelines, limiting macro and micronutrient intake and contributing to the intergenerational malnutrition cycle. Additional research in the country is essential in order to identify other intake patterns and drive political action.

SUMMARY OBJECTIVE: This study aimed to determine the validity, structure, and reliability of a Turkish version of the Fathers' Breastfeeding Attitude and Participation Scale. The Fathers' Breastfeeding Attitude and Participation Scale consists of two parts, namely, Fathers' Breastfeeding Attitude and Fathers' Participation in Breastfeeding Process. Totally, the scale consists of 28 items, of which 14 items belong to Fathers' Breastfeeding Attitude and 14 items belong to Fathers' Participation in Breastfeeding Process. There is no report of a validity and reliability study in the original scale. Currently, there is no validated and reliable scale to assess Fathers' Breastfeeding Attitudes and Involvement in Turkish literature and other languages. In this context, the psychometric properties of the Fathers' Breastfeeding Attitudes and Involvement Scale were examined. METHODS: The instruments were translated and adapted according to the WHO guidelines. RESULTS: The Turkish version of the Fathers' Breastfeeding Attitude and Participation Scale demonstrated acceptable validity and reliability. CONCLUSION: The use of the validated instrument to examine fathers' breastfeeding attitudes and participation in the breastfeeding process will provide data to guide as it is a determinant of breastfeeding behavior.

Abstract The return to work of lactating mothers has been identified as an important risk factor for breastfeeding interruption. We proposed to identify factors associated with breastfeeding abandonment in the first month after return. 252 women working at a hospital who had children aged 12 to 36 months and who were still breastfeeding when returned to work answered a questionnaire containing questions on sociodemographic characteristics, pregnancy, childbirth, breastfeeding and work-related factors. The associations were estimated using adjusted prevalence ratio (aPR), calculated with the hierarchical Poisson multivariable regression model. The following factors showed a significant association with breastfeeding abandonment: using a pacifier (aPR 4.58), cohabiting with someone other than partner (aPR 3.77), having no intention or having doubts about maintaining breastfeeding after returning (aPR 3.39), having a college degree (aPR 2.66), having no support from the infant's caregiver (aPR 2.26), and infant being older when the woman returned to work (PR 1.16 for each additional month of infant age). Longer duration of exclusive breastfeeding was a protective factor (aPR 0.990). Most of the factors associated with discontinuation of breastfeeding in the first month after the mother's return to work are not directly related to the woman's work.


Resumo O retorno da lactante ao trabalho é apontado como importante fator de risco para interrupção da amamentação. Nós nos propusemos a identificar fatores associados ao abandono da amamentação no primeiro mês após o retorno. Compuseram a amostra 252 servidoras de um hospital com filhos entre 12 e 36 meses e que estavam amamentando quando retornaram ao trabalho, fornecendo informações sobre características sociodemográficas, gestação, parto, amamentação e fatores relacionados ao trabalho. As associações foram estimadas pela razão de prevalência ajustada (RPa), obtida usando modelo hierárquico de regressão multivariada de Poisson. Os seguintes fatores mostraram associação significativa com abandono da amamentação: uso de chupeta (RPa 4,58), coabitação com outra pessoa que não o companheiro (RPa 3,77), não ter intenção ou ter dúvidas sobre amamentar após retorno (RPa 3,39), ter curso superior (RPa 2,66), não ter apoio do cuidador da criança (RPa 2,26) e maior idade da criança quando a mãe retornou ao trabalho (RPa 1,16 para cada mês de idade a mais da criança). Maior duração da amamentação exclusiva foi fator de proteção (RPa 0,990). A maioria dos fatores associados à descontinuidade da amamentação no primeiro mês após retorno da lactante ao trabalho não está diretamente relacionada ao seu trabalho.

Background: Breastfeeding (BF) is considered one of the basic pillars to promote and protect children's health and its social impact can be assessed through reduced medical consultations. Methods: This qualitative, non-empirical study with a sample of 200 mothers and a cross-sectional descriptive survey to achieve the set objectives. The study will be conducted at a selected hospital in Bagalkot. An open and closed structured questionnaire collected information from nursing mothers. Result: The distribution of the proportion of mothers by socio-demographic among 200 mothers had the highest age-based ratio of 46% in the 24-29 age group. 56% high school level. 73% of mothers are housewives, 57 years old. The delivery method is 62% of full-term vaginal delivery. According to the program, 80% when crying normally and 64.5% when feeding. Birth weight is 49.5%, if from 1000 to 2500 g, a statistically significant relationship exists between baby's behavior x2=0.41, breastfeeding pattern x2=0.87, birth weight of children x2=0.02, p>0.05. Conclusion: The study results showed a significant positive relationship between mothers' breastfeeding effectiveness and participants' confidence. This factor can help healthcare providers predict the period of breastfeeding and the success rate of exclusive breastfeeding in mothers, thereby identifying mothers at risk of stopping breastfeeding. Early breastfeeding.

Resumen Introducción: la superioridad de la leche humana (LH) por sobre cualquier otro alimento en los primeros seis meses de vida, ya ha quedado bien demostrado a través de numerosos estudios científicos. Resulta importante investigar e indagar sobre el grado de conocimientos previos con los que cuentan las madres, con el fin de definir abordajes para futuras intervenciones. Objetivo: determinar el nivel de conocimiento sobre lactancia humana que poseen las madres puérperas que se encuentran en el área de internación de maternidad del H.Z.G.A Dr. A. Balestrini, Ciudad Evita, provincia de Buenos Aires. Materiales y método: estudio observacional, descriptivo y transversal. Realizado entre los meses de febrero y mayo inclusive del año 2021. Se estimó el conocimiento sobre LH a 134 puérperas que se encontraban internadas en la sala de maternidad a través de una entrevista estructurada realizada a pie de cama por licenciadas en Nutrición. Resultados: la media de edad fue de 25,8 (DE±5,5) años. El promedio de hijos fue 2,3. Más del 80% reconocía los beneficios de la LH para los recién nacidos. Sin embargo, solamente el 35,8% reconocía los beneficios de la LH para la madre. El 48,5% contestó que no existían riesgos para el recién nacido respecto del consumo de leche de vaca (LV). Las creencias populares más prevalentes fueron "el dolor durante la lactancia es normal" y "me quedé sin leche". Conclusión: se reconocían las prácticas generales de la lactancia como su exclusividad hasta los 6 meses y continuación hasta los dos años. Por otro lado, se observó que las mujeres identificaron los beneficios que otorga la LH al bebé y la superioridad de la misma sobre la LV. Sin embargo, esto no sucedió con aquellos beneficios de la LH para las puérperas y el riesgo del uso de la LV en el recién nacido.


Abstract Introduction: the superiority of breast milk (BM) over any other food in the first six months of life has already been well demonstrated through numerous scientific studies. It is important to research and inquire about the degree of prior knowledge that mothers have in order to define approaches for future interventions. Objective: to determine the level of knowledge about breastfeeding among postpartum mothers in the maternity ward of the H.Z.G.A. Dr. A. Balestrini, Ciudad Evita, province of Buenos Aires. Materials and method: observational, descriptive and cross-sectional study, carried out between February and May 2021. Knowledge about BF was estimated to 134 puerperal women, who were hospitalized in the maternity ward, through a structured interview conducted at the bedside by graduates in Nutrition. Results: the mean age was 25,8 (SD±5.5) years. The average number of children was 2.3. More than 80% recognized the benefits of BF for the newborns. However, only 35.8% recognized the benefits of BF for the mother. 48.5% answered that there were no risks for the newborn with respect to cow's milk consumption (CM). The most prevalent popular beliefs were "pain during breastfeeding is normal" and "I ran out of milk". Conclusion: it was evidenced that the exclusivity of BM up to 6 months and its continuation up to two years were recognized. On the other hand, it was observed that women identified the benefits granted by BM to the baby and its superiority over CM. However, this didn't happen with the benefits of BM for postpartum women and the risk of using CM in the newborn.

Introduction: Healthy initiation of life is essential in establishing the foundations of a healthy nation. Traditional care practices at home and in the community inevitably affect maternal and young infant health. The objective is to study young infant nurturing practices and factors influencing these practices in rural Agra. Material and Methods: This is a community-based cross-sectional study that was conducted in rural Agra, using a multi-stage random sampling method. A total of 240 mothers were interviewed using a structured questionnaire. Results: The study results showed that 65.0% of the mothers were following either satisfactory (58.3%) or good (6.7%) nurturing practices. Of the three components of nurturing, the majority (61.6%) of mothers had satisfactory (46.6%) or good (15%) breastfeeding practices, most (86.6%) were giving satisfactory (56.6%) or good (30%) thermal care and only 53.33% were following satisfactory (42.2%) or good (10.8%) hygiene practice. The practice regarding newborn care was found to have a significant association with the place of birth, type of delivery and educational status of the mothers and fathers. Conclusion: This study outcome shows the need for a better awareness and educational outreach program coupled with an effective health-care delivery system to improve young infant nurturing practices in achieving better health.

Introdução: Durante o período da gravidez e da amamentação a mulher vivência mudanças fisiológicas, psicológicas e de ordem estrutural, essas mudanças podem trazer alterações na sexualidade feminina, devendo os profissionais de saúde ter uma atenção especial as suas necessidades particulares e com direitos sexuais. Objetivo: Conhecer as representações que os acadêmicos de enfermagem possuem acerca da sexualidade durante a amamentação. Método: Trata-se de uma pesquisa qualitativa, descritiva e exploratória. Participaram da pesquisa 16 acadêmicos de Enfermagem de uma universidade pública do interior da Bahia, Brasil. Para coleta dos dados empíricos foi utilizado à técnica projetiva de desenho-estória com tema e analisados mediante a técnica de análise de conteúdo. Resultado: Após análise, revelaram-se três categorias: sentimentos e desejos sexuais adormecidos devido à atenção dispensada para o bebê e os afazeres domésticos; sexualidade representada pela demonstração de amor, apoio, companheirismo, dedicação e respeito à mulher durante o período de amamentação e interferências na sexualidade permeada pelo comprometimento da autoimagem e autoestima da mulher. Conclusão: Conclui-se que a amamentação e a sexualidade foi percebida pelos acadêmicos de enfermagem através de conceitos estabelecidos socialmente, vivenciados ou não, assim é de extrema importância à abordagem desse tema na formação dos profissionais de saúde, para que possam descontruir alguns tabus enraizados durante a sua construção social.


Introducción: Durante el período de embarazo y lactancia, las mujeres experimentan cambios fisiológicos, psicológicos y estructurales. Estos cambios pueden provocar cambios en la sexualidad femenina, por lo cual las personas profesionales de la salud deben prestar especial atención a sus necesidades particulares y derechos sexuales. Objetivo: Conocer las representaciones que la población estudiantil de enfermería tiene sobre la sexualidad durante la lactancia. Método: Se trata de una investigación cualitativa, descriptiva y exploratoria. Dieciséis estudiantes de Enfermería de una universidad pública del interior de Bahía, Brasil, participaron de la investigación. Para recolectar los datos empíricos, se utilizó la técnica proyectiva de dibujo-cuento con tema y se analizó mediante la técnica de análisis de contenido. Resultado: Después del análisis, se revelaron tres categorías: sentimientos y deseos sexuales latentes debido a la atención al bebé y las tareas del hogar, la sexualidad representada por la demostración de amor, apoyo, compañerismo, entrega y respeto a la mujer durante el período de lactancia, e interferencia en la sexualidad permeada por el compromiso de la propia imagen y autoestima de la mujer. Conclusión: Se concluye que la lactancia materna y la sexualidad fueron percibidas por las personas estudiantes de enfermería a través de conceptos socialmente establecidos, experimentados o no. Por lo que, es de suma importancia abordar este tema en la formación de las personas profesionales de la salud, para que puedan deconstruir algunos tabúes arraigados durante su desarrollo social.


Introduction: During the period of pregnancy and breastfeeding, women experience physiological, psychological, and structural changes; these changes can impact their sexuality. Health professionals should pay special attention to their particular needs and sexual rights. Objective: To understand the perception that nursing students have about sexuality during breastfeeding. Method: This is qualitative, descriptive, and exploratory research. Sixteen nursing students from a public university in the interior of Bahia, Brazil participated in the research. To collect the empirical data, the projective technique of drawing a story about a given theme was used and the result was analyzed using the technique of content analysis. Result: After the analysis, three categories were revealed: dormant sexual feelings and desires due to the attention given to the baby and household chores; sexuality represented by the demonstration of love, support, companionship, dedication, and respect to women during the period of breastfeeding; and sexuality interference permeated by the commitment of the woman's self-image and self-esteem. Conclusion: It is concluded that breastfeeding and sexuality were perceived by nursing students through socially established concepts, experienced or not. Therefore, it is extremely important to approach this topic during the training of health professionals to help them deconstruct those socially ingrained taboos.

Resumen La alimentación durante los primeros 1000 días de vida (desde el vientre materno hasta los dos años) son fundamentales para la salud futura de una persona. Este artículo tiene por objetivo analizar los saberes y prácticas alimentarias materno-infantiles en localidades rurales del Estado de Hidalgo en México. Estudio de corte transversal, descriptivo, observacional desde una perspectiva fenomenológica combinando técnicas cuantitativas y cualitativas en 56 participantes y 7 informantes a quienes se entrevistó. El 48% de las mujeres entrevistadas ofrecieron leche materna exclusivamente durante 4 a 6 meses, el 57% destetó después del año. La familia fue la principal fuente de información de las mujeres (91%), por encima del personal de salud (80%), quien frecuentemente da opiniones contradictorias. El 71% prefirió el uso de conocimientos de herbolaria provenientes de parteras, hijas de parteras o adultas mayores. Los saberes, tradiciones y prácticas alimentarias de las madres de menores de dos años en el Carso Huasteco hidalguense, se basan en conocimientos tradicionales permeados por el discurso del personal de salud y los medios de comunicación, así como el acceso físico y económico a los alimentos procesados y frescos.


Abstract Nutrition during the first 1000 days of life (from the womb to two years of age) is fundamental for the future health of a person. The objective of this article is to analyze maternal and child feeding knowledge and practices in rural localities of the state of Hidalgo in Mexico. A cross-sectional, descriptive, observational study was carried out from a phenomenological perspective combining quantitative and qualitative techniques in 56 participants and 7 informants who were interviewed. 48% of the women interviewed offered breast milk exclusively for 4 to 6 months, 57% weaned after one year. The family was the main source of information for the women (91%), above the health personnel (80%), who frequently gave contradictory opinions. Seventy-one percent preferred the use of herbal knowledge from midwives, daughters of midwives, or older women. The knowledge, traditions, and food practices of mothers of children under two years of age in the Carso Huasteco hidalguense are based on traditional knowledge permeated by the discourse of health personnel and the media, as well as physical and economic access to processed and fresh foods.
